“In my case, I think about the world setting, then create the story. The character settings are pretty vague at first. The details aren't refined yet.”
What it documents

Kojima’s stated order of construction, confessed with Kutsuna present, is that he first thinks about what he should do with certain movie stars and sometimes decides a character’s approach based on the actor. In his case he thinks about the world setting and then creates the story; the character settings are pretty vague at first and the details are not yet refined. They then digitalize those ideas, and in that process the story can change, which naturally changes the gameplay, so they fix things up as they go. Once an actor is decided, for example Shioli, Kojima incorporates the impression he got from her — gestures and habits — into the character. This also occurs during captures: small movements and gestures that arise are kept. He does this work together with his actors.

[…] And from there, we digitalize these ideas, and in that process, the story may change. Which changes the gameplay, naturally, so we fix it up as we go. And once the actors are decided on, for example, Shioli, I incorporate the impression I got from her, like any gestures and habits, into her character.”
